Osteoporosis Canada (OC) is a provincial organization that supports over two million Canadians nationally affected by osteoporosis. Focused on education, empowerment and support, OC uses these pillars to uplift and inform individuals and communities on bone health, risk reduction and the treatment of osteoporosis. OC strives to reduce the risk of hip and other osteoporotic fractures, one day envisioning a fracture-free Canada..
THE PROBLEM
Patients who suffer from osteoporosis are at significant risk of bone fractures. With a goal to reduce the number of osteoporosis-related fractures, OC required a platform to collect and analyze data to support this initiative. A transition to a modern, cloud-based platform was required to formalize the management of patient data. In addition, as OC works in collaboration with clinical practitioners, they require a method to measure the effectiveness of ongoing treatments and tailor future care based on patient receptiveness and efficacy.
THE SOLUTION
A secure, confidential and centralized screening database with dynamic surveys, and easily accessible mobile app.
The primary goal of Osteoporosis Canada is to reduce fragility fractures and re-fractures through early detection and appropriate intervention. OC working in collaboration with Naryant developed the Fracture Clinic Screening Database which is based upon the Canvass Health platform. This database allowed OC to securely and confidentiality track and maintain relevant data in a centralized location. The Naryant tool allowed OC to also securely access patient data and obtain insightful and dynamic surveys designed to analyze the data through a powerful reporting engine. Naryant has also provided OC with a mobile app solution to create an accessible approach to accessing clinical data.

THE RESULTS
A highly configurable and flexible registry with advanced features.
Used by 25 Fracture Prevention Coordinators (FPC) across 36 fracture clinics, the FSPP assists in the screening of fragility fractures and facilitates diagnosis and appropriate care. Used alongside diagnostic and assessment tools, this platform also aids in identifying at-risk patients and fracture risk assessment.
